NightmareSchool is classified as a "reverse escape" Action RPG (ARPG). This means that instead of the player chasing enemies, the enemies are constantly chasing the player. The gameplay loop is tense and straightforward. The player must navigate the school's hallways and classrooms while avoiding roaming packs of aggressive students. The students will pursue the protagonist on sight, and the challenge lies in using the environment to break their line of sight, finding hiding spots, and slipping past them to progress.

This is not a game for everyone. The adult themes are integral, not optional. If you dislike “game over as punishment” mechanics, the steep difficulty curve of the Final version’s Despair mode will frustrate you. Additionally, the English translation (by a fan group) is serviceable but leaves some Japanese horror puns untranslated.
